Entry fee is a blanket that can be dropped off at the Chamber of Commerce or brought on the day of the parade.

You can submit your application below or download the application and submit it to forsythareachamber@gmail.com, drop it off at 157 Main Street, Forsyth, or mail it to P.O. Box 777, Forsyth, MO 65653.
